Flange Fitting in the NICU to Improve Pumping Outcomes

Abstract:

Incorrectly sized breast pump flanges can negatively impact breastfeeding outcomes, including reduced milk yield, increased maternal discomfort, and heightened risk of mastitis. Research (Anders, Mesite Frem, and McCoy, 2024) highlights that correctly measured and fitted flanges significantly improve outcomes for both mothers and infants. Despite this, nipple measurement and individualised flange sizing is not standard practice, and most hospitals only stock a standard flange size, which is often much too large for the majority of pumping parents. Consequently, many parents must purchase appropriate flanges themselves, disproportionately affecting those from low-income backgrounds and delaying more effective expression of maternal colostrum and breastmilk.
This quality improvement project aimed to improve equity of access to correctly fitting breast pump flanges across NHS Trusts in the Wessex region in the United Kingdom. The project sought to raise awareness among staff and parents of the importance of correct flange sizing, provide appropriate tools for flange fitting, and ensure availability of a range of flange sizes within hospitals.
This initiative has demonstrated early success in improving awareness, education and access to appropriately sized breast pump flanges across the Wessex region, contributing to increased breastmilk provision for vulnerable and premature infants, as well as for term babies. The initiative promotes health equality and early results suggest a measurable impact on clinical outcomes.
