Nipple Shields in the NICU: Evidence, Indications, and Best Practices

Abstract:

Feeding in the NICU presents unique challenges for both infants and families. Preterm and medically complex neonates often struggle with immature suck–swallow–breathe coordination, hypotonia, and fatigue, while mothers may face anatomic barriers, delayed lactogenesis, or stress-related supply issues. Nipple shields are sometimes introduced in this context as a bridge to more effective breastfeeding—but their use remains controversial. This presentation explores the appropriate role of nipple shields in the NICU, drawing on current evidence to examine their impact on milk transfer, breastfeeding duration, maternal confidence, and infant outcomes. Through a critical review of the literature, participants will learn when nipple shields may be indicated, when to avoid them, and how to implement safe and effective protocols for sizing, fit, infection control, and documentation. Case-based scenarios will highlight practical decision-making in preterm, late-preterm, and medically complex dyads. Emphasis will be placed on strategies for weaning, protecting maternal supply, and measuring outcomes to ensure nipple shields remain a tool rather than a crutch. Attendees will leave with research-informed best practices and practical algorithms to guide clinical care in the NICU.
