A Multi-disciplinary Approach to Assessing Dysphagia While Protecting Breastfeeding

Abstract:
Medically complex and premature infants are at a higher risk for dysphagia. While these infants' swallowing skills can be evaluated through a VFSS (Video-Fluoroscopy Swallowing Study) also know as an MBSS (Modified Barium Swallow Study), this type of study necessitates bottle feeding. Infants who refuse bottle feeding, or families whose preference is direct breast/chestfeeding, are unable to be evaluated in this way. A creative multi-disciplinary approach can be taken by involving an IBCLC (International Board Certified Lactation Consultant, a SLP (Speech Language Pathologist), and an ENT (Ear, Nose, and Throat) physician. The lactation parent can latch and directly breastfeed while a FEES (Fiberoptic Evaluation of Swallowing) study is done to assess swallowing skills. This flexible approach serves to meet parent goals while allowing the care team to make real time changes and create a safe feeding plan.
