Preterm and late preterm infants have unique needs when it comes to feeding. Due to the fact that they are born before their brain, nervous system and body have had a chance to fully mature, they may have issues with things like suck-swallow-breathe coordination, poor muscle tone and disorganized sleep-wake cycles, all of which have an impact on an infant's ability to breastfeed effectively. This puts them at greater risk for complications such as slow weight gain, hypoglycemia, jaundice and failure to thrive. Common reasons for this include a baby who is unable to effectively remove milk from the breast or a parent who is not producing enough milk to meet the baby’s needs. A nursing supplementer – or supplemental feeding tube device as it is often referred to in journals – can serve as an “external milk duct” providing plenty of extra flow when the baby needs an easier flow or parents need an increased flow. A nursing supplement can help preserve the breastfeeding relationship and increase milk production. So why isn’t it used more often? It can be challenging to use, especially at first. As a lactation consultant who specializes in working with parents inducing lactation, Alyssa has seen the struggles and successes of using a nursing supplementer first-hand. In this presentation, she shares her professional experience and the wisdom of the research to help other lactation professionals support clients with this incredible tool for supporting long-term breastfeeding outcomes.

Chesney Willis is a certified speech-language pathologist and lactation consultant with over fifteen years of clinical experiencing, specializing in feeding and swallowing disorders in the outpatient clinical setting. She is affiliated with the University of Missouri Health Care system, and treats infants, toddlers and young children with histories of complex feeding difficulties. Specialties include NICU graduate follow up care, breastfeeding and bottle feeding challenges for infants with special feeding needs, g-tube management and weaning, and picky/avoidant eating. In addition to the clinical setting, Chesney also completes diagnostic modified barium swallow studies and serves on the University of Missouri Children’s Aerodigestive Team.
Prior to transitioning to the outpatient clinical setting, Chesney spent the majority of her career working with complex neonates in the NICU. She hopes to function as a liaison between the families she serves and health care practitioners to provide safe, seamless, and enjoyable feeding experiences for children and their families. Abstract:View Full Presentation InformationIn this session, participants will identify newborns’ typical feeding and swallowing development and the impact of prematurity on these early processes. While premature infants and babies with special needs may transition to full oral feeding at a slower rate, early coordination of the swallow may emerge as early as 32 to 34 weeks depending on the degree of prematurity and underlying comorbidities. Despite the early emergence of developmental reflexes, the transition to full oral feeding among infants born prematurely may take longer than term infants without additional health barriers. Given such variability, evidence-based and systematic clinical decision-making is required to promote improved infant and family-centered long-term outcomes. As successful feeding requires recognizing an infant’s cues, adjusting a feeder’s response, and adapting the experience via supportive modifications for safe and efficient intake, this session will focus on the knowledge, skills, and abilities to support this process. Participants will learn to identify clinical signs of dysregulation associated with aspiration, negative pulmonary implications, and/or maladaptive feeding patterns as well as how to teach responsive and developmentally supportive feeding strategies. Finally, community resources centered on supporting individuals caring for infants with feeding difficulties will be highlighted in efforts to mitigate complications and enhance follow-up care.

Constance Banks has been an international board-certified lactation consultant since 1996 and a registered midwife since 1993. She lives in Melbourne Australia. Originally from Germany, where she trained as a nurse, Constance has worked in other countries and has a passion and interest for other cultures and their childrearing styles. She worked as a lactation consultant and clinical educator for 10 years in an associate management role at a large teaching hospital, where she also covered work in the milk bank. In the past Constance was an educator and assessor for the Baby Friendly Health Initiative. In June 2022 she resumed running a small private lactation practice under the name “Nurture Heart and Soul” Breastfeeding Support.

It’s important to understand that key pieces of this transition need to happen right from the start of the infant’s time in the NICU. Skin-to-skin contact and non-nutritive sucking at the breast help to lay the foundation for successful breastfeeding. This presentation will guide the learner through the full journey of the transition from bottle to breast, starting with skin-to-skin contact and establishing robust milk production, and including common oral challenges and approaches to management and setting families up with a discharge plan for successful breastfeeding.

Heather Latiolais Eure is an IBCLC in Austin, Texas, USA. She is the owner of Oxytocin Perinatal Support, through which she offers lactation care, postpartum doula support, and perinatal behavioral health coaching. Heather became involved in lactation support when she attended her first La Leche League meeting in 2012 during her pregnancy with her oldest child. She was accredited as a La Leche Leader in 2015. Shortly after learning she was pregnant with her third child, she decided to turn her passion for breastfeeding into a career and became an IBCLC in 2020. Before she entered the field of lactation, Heather completed a PhD in Comparative Literature with a certificate in Women’s and Gender Studies from The University of Texas and taught college-level French and writing courses.

In this presentation, I shed light on the particular mental health risks that parents of preterm and late preterm babies face. Parents of such babies experience health complications and infant feeding difficulties at a higher rate than their term counterparts; furthermore, they are thrust into their new identities as parents often much earlier than expected; all of these factors can exacerbate mental health challenges in this population. Research illustrates a bidirectional relationship between parental mental health and breastfeeding success; consequently, feeding is a critical point of focus in the care of these dyads to improve the health of both baby and parent. While the extent to which parental mental health can affect this specific population of infants is unclear, untreated perinatal mood and anxiety disorders (PMADS) increase the risk of a host of problems from exacerbation of medical conditions to developmental delays. Parents of preterm and late preterm babies therefore warrant targeted screening and specialized support. Learn how to take a family-centered approach to care which includes regular screening for PMADs and consistent support throughout the postpartum period.

Dr Mark Johnson is a consultant neonatologist at University Hospital Southampton NHS Foundation Trust and an honorary senior clinical lecturer in neonatal medicine at the University of Southampton. He is also currently the clinical director for neonatal medicine in Southampton. Dr Johnson graduated in medicine (BM) from the University of Southampton in 2004, and undertook an intercalated degree in biomedical science during his medical training.
Dr Johnson was awarded a prestigious NIHR doctoral research fellowship to fund his PhD, which he was awarded by the University of Southampton in 2015.Dr Johnson’s PhD focused on change management in neonatal care in the context of nutrition, successfully implementing improved nutritional practices in order to improve the growth of preterm infants.
Dr Johnson's research centres around the nutritional care and growth of premature babies, and the implementation of practice change in order to improve care, and has published on these topics widely, including systematic reviews looking at the use of early parenteral nutrition in preterm infants, the impact of enhanced nutrition on the neurodevelopmental outcomes of preterm infants and the effect of preterm birth on body composition and growth. Abstract:View Full Presentation InformationCurrently, one of the goals of preterm infant nutritional care is to try and achieve rates of growth and patterns of body composition similar to those seen in utero at the equivalent gestation. This is challenging as it requires much higher amounts of nutrition, than that required by term infants, which can be hard to deliver to preterm infants in neonatal intensive care. Breast milk is the best milk for preterm infants, with advantages including reduced risk of necrotising enterocolitis and improved microbiome. However, it does not contain sufficient amounts of key nutrients, including energy, protein, and electrolytes, to meet the needs of preterm infants. Breast milk fortifier is therefore added to improve its nutrient content. This talk will look at current growth targets for preterm infants, their nutritional requirements, and how to achieve these using breast milk in conjunction with fortifier. It will look at the current evidence for fortifier use, concerns around its use, and the timing of administration. Different approaches to fortification will also be discussed, including adjusted and targeted fortification, the use of fortifiers beyond hospital discharge, and the use of fortifiers made from human milk rather than cow’s milk.

This presentation will discuss the etiology and prevalence of late preterm deliveries and morbidities that impact this population. We will also discuss sensory development and its impact on feeding skill development as well as strategies that focus on providing preventative and proactive family centered care.
